We request copayments at the time off service. We do accept credit cards.
Do call our office to make an appointment. Naturally, all forms are available at the practice.
If you have any questions about your insurance or are unsure if it reimburses, don't hesitate to contact us.
Note: You will not be rescheduled if we feel that your account with is outstanding for over a month.
